Evaluators Evaluation Form

For 2024 We are trying to get evaluations on everyone. Here is the form your evaluator will be sending you as an umpire. If your evaluator doesn't get back to you within a few days call them or call the Pres. or VP.

Evaluators: This form is designed to evaluate all the umpires in a game on one form. If you use this form, you must transcribe it to the "Umpire Evaluation Form" and send a copy of that form to the individual umpires. All hard copies of this or any evaluation form from the evaluator MUST be given to the President or VP PLEASE!

Evaluators please go over the transcribed Umpire Evaluation Form with the umpires you evaluate with in a few days. Send them a picture of the front and back.
