Umpire Evaluation Form

For 2024 We are trying to get evaluations on everyone. Here is the form your evaluator will be sending you as an umpire. If your evaluator doesn't get back to you within a few days call them or call the Pres. or VP.

Evaluators: This is the form you will send to the individual umpires you evaluate. The hard copy of this form from the evaluator MUST be given to the President or VP PLEASE!

Evaluators please go over this form with the umpires you evaluate with in a few days. Send them a picture of the front and back.
